     T-Shirt/Sweatshirt preorder forms:   April 6th
     Deadline for Roster Changes:     April 22nd
     Tournament Check-In:    Friday, May 1st, 6pm-9pm

CHECKLIST FOR REGISTRATION:

* Complete the on-line tournament registration (print copy of application)
* Complete the on-line roster         
* Mail the Following 4 items to:
               Tournament Registrar
               2020 AYSO Pacifica Fog Classic
               936 Lincoln Place
               Pacifica, CA  94044

    1. Tournament Application ( a printed copy of your application from the gotsoccer website 
         signed by your Regional Commissioner)
    2. Referee Information Form (signed by your Regional Referee Administrator)    
    3 . Official Blue Sombrero Roster (signed by your Regional Commissioner) 
    4 . A Regional Check made out to "AYSO 157" for:

                               U-10                 $525*            
                               U-12                 $600*            
                               U-14/16/19      $650*  

                                *Includes referee deposit

FOR COACHES:  your "APPLICATION DATE" 
will be the day we receive your COMPLETED application materials, NOT the date you fill out your on-line registration.
